Telehealth Consent

What telehealth is

Telehealth is the delivery of healthcare services using electronic communication — secure questionnaires, messaging, and where used, phone or video — when you and the provider are not in the same place. Through Risen MD, care is delivered remotely by independent, licensed U.S. providers licensed in the state where you are located at the time of care. Risen MD itself does not practice medicine.

What to expect

  • You complete a private intake. A licensed provider reviews it and may ask for more information, laboratory results, a phone or video visit, or an in-person evaluation before deciding anything.
  • The provider decides whether any treatment is appropriate for you. A prescription is never guaranteed, and the provider may decline to treat you or refer you to in-person care.
  • The provider will identify themselves to you by name and credential. You may ask about their licensure at any time.
  • You may message the provider who treated you through your secure patient portal. Your provider communicates with you through that portal; email and text notifications tell you a message is waiting and do not contain its content.

Benefits and limits

Telehealth can make care more convenient and private. It also has limits: the provider cannot examine you physically; information transmitted may not be sufficient for a decision in every case; delays or technical failures can occur; and some conditions require in-person care. If the provider determines that telehealth is not appropriate for you, they will tell you and direct you to appropriate care.

Not for emergencies

If you may be having a medical emergency, call 911 or go to the nearest emergency department. Do not use the intake or portal for urgent symptoms.

Medications

Any medication prescribed is dispensed by a licensed U.S. pharmacy. Some programs use compounded medications, prepared for you individually by a licensed compounding pharmacy under your prescription; compounded medications are not FDA-approved. Your provider will discuss risks, side effects and alternatives with you and you may ask questions at any time.

Privacy and security

Your clinical information is collected and stored inside a HIPAA-compliant platform and handled as described in the Notice of Privacy Practices. Electronic communications carry some risk of interception despite the safeguards used; you agree to the use of these electronic systems for your care.

Your rights

  • You may withhold or withdraw this consent at any time without affecting your right to future care, by emailing support@risenmd.com.
  • You may request a copy of your records as described in the Notice of Privacy Practices.
  • You will not be denied care for declining telehealth; the alternative is in-person care from a provider of your choice.

Consent

By checking the consent box in the intake you confirm that you have read and understand this consent, that you are at least 18, that you are located in a state we serve, that the information you provide is accurate and complete, and that you consent to receive care by telehealth from an independent licensed provider on the terms above.
